• Lab Online Booking is not a healthcare provider
• All healthcare services are provided solely by independent healthcare providers

You understand that any medical care, outcomes, or decisions are the sole responsibility of the healthcare provider.

The Platform must not be used for urgent or emergency medical needs.

If you are experiencing a medical emergency, call 911 or attend the nearest emergency department.

This Platform is designed for administrative booking purposes only.

You agree that you will not submit, upload, or enter:

• symptoms
• diagnoses
• test results
• medical history
• clinical notes
• or any other health-related information

You acknowledge that:

• The Platform is not intended to collect or process medical information
• Any information entered is not reviewed for clinical purposes
• Any health-related information submitted may be deleted without notice

Lab Online Booking:

• does not store medical records
• is not a health information custodian
• does not maintain or manage clinical data

All medical information is managed directly by the healthcare provider.

Healthcare providers using the Platform:

• are independent entities
• are not employees, agents, or representatives of Lab Online Booking
• are solely responsible for their services, operations, and compliance

Lab Online Booking does not:

• control clinical decisions
• guarantee appointment availability or outcomes
• endorse or recommend any provider

You agree that:

• You are responsible for providing accurate booking information
• You will communicate all medical or health-related information directly with your healthcare provider
• You will comply with provider-specific policies (including cancellations and no-show policies)

By using the Platform, you consent to receive:

• appointment confirmations
• reminders
• service-related notifications

You may optionally opt-in to receive additional communications. These are not based on medical or health information.

Lab Online Booking collects limited administrative information required to facilitate bookings, including:

• name
• contact information
• appointment details

You acknowledge that:

• Your personal information is used solely for operational and platform-related purposes
• Lab Online Booking may use aggregated and de-identified data to improve services, analytics, and system performance
• Lab Online Booking does not sell or share identifiable personal or health information

To the maximum extent permitted by law:

Lab Online Booking is not responsible for:

• healthcare services provided by providers
• medical outcomes or treatment decisions
• appointment availability or provider actions

You agree that your use of the Platform is at your own risk.

You agree to indemnify and hold harmless Lab Online Booking from any claims arising from:

• your misuse of the Platform
• submission of prohibited information (including medical or health information)
• violation of this Agreement

By selecting "I Agree" or using the Platform, you confirm that:

• You have read and understood this Agreement
• You agree to comply with all terms
• You acknowledge the Platform is non-clinical, administrative only
